Records request for an Incident Report , Accident Report or Body-Worn Camera Footage**

There are four (4) different ways to request a copy of a police report, accident report, and/or body-worn camera footage:

  1. Click here to fill out a request form and submit online. 
  2. Fill out this form and mail it to:

    New Haven Department of Police Service
    Attention: Records Unit
    1 Union Avenue
    New Haven, CT  06519
  3. You may also bring this form to the New Haven Department of Police Service and present to the Front Desk Sergeant.
  4. You may also fax this form to (203) 946-6274.

**Requests will be processed in the order they are received and can take upto 14 days.  You will be contacted by the Records Unit with the cost of your request.**  

NHPD REPORT FEES:

  • There is a fifty-cents ($0.50) per page fee for each report requested. Reports 10 pages or less can be emailed free of charge.
  • The programming fee for body camera footage requested shall be: fifty ($50) for the first two hours and ten ($10) for each additional hour.* Updated Nov 2020
  • The fee for complete accident reconstruction reports is $525.

Payments will be accepted in the form of money orders and checks made payable to the Treasurer, City of New Haven.

Please provide as much information as possible when requesting the report or footage:

  • Full names of parties involved
  • Date of incident
  • Time of incident
  • Location of incident
  • Type of incident (burglary, vandalism, traffic accident, etc.)
  • Report number

*** Please note that the release of information is dictated by State Statute.  Some incidents, particularly open investigations, may not be releasable.  Some information, also dictated by State Statute, may be redacted from a report or body-worn camera footage.

There are four (4) different ways to request a New Haven Arrest Record Check:

Requests will be processed in the order they are received.  There is a five dollar ($5) charge per background requested.  If you submit the form through email or fax, you must mail payment for your request to be processed as well as a copy of the request form.  Payments will be accepted in the form of money orders and checks made payable to the Treasurer, City of New Haven.

*** Please note that this is a search for New Haven arrests only.  You will be provided with information on whether there is an arrest but will not be provided with the record (you may request this record separately, see above). If you need a full State of Connecticut Criminal Conviction History, click here.
